Required Forms

If you are a dealer that needs to adjusts or change an existing license then you will need to report a name or address change or any other changes on your license by completing a HSMV 86720 which can be found at:  Renewal Application

Verify a Vehicle Identification Number or the odometer reading verification of a used vehicle that is being titled in the State of Florida for the first time requires a H82042 form which can be found online. A form DHSMV 82995 will need to be used in cases that you need to obtain a power of attorney.
